Checklist · Updated 2026-09-29

Tasks for the weeks before, the day of, and the weeks after your parent moves into assisted living: paperwork, packing, moving day and staying involved.

Use this checklist over the weeks leading up to the move. Start with the paperwork section, since some documents and assessments can take time to arrange.

Keep a copy on the fridge or in a shared family folder so everyone helping can see what is done. For more detail on each step, see our guide to [preparing for the move](https://assistedlivingguides.com/guides/preparing-to-move/index.md).

## Paperwork

- [ ] Read the residency agreement fully and asked about anything unclear
- [ ] Asked the community for its list of required move-in documents
- [ ] Scheduled any health assessment or tests the community requires
- [ ] Prepared a current medication list with doses and prescribing doctors
- [ ] Gathered insurance cards and copies of power of attorney, health care proxy or advance directives

## Downsizing and packing

- [ ] Measured the new apartment and got a floor plan
- [ ] Asked what the community provides and what is not allowed
- [ ] Chose favorite furniture, photos and familiar items with my parent
- [ ] Labeled clothing and packed glasses, hearing aids and chargers

## Moving day and after

- [ ] Confirmed move-in time and who will meet us
- [ ] Learned who my main point of contact is for questions and concerns
- [ ] Asked when the first care plan meeting will be held

Costs, rules and services vary by state and by community. Use this site to prepare, then confirm details with the community and your state licensing agency.
